Do I need a referral? Does insurance cover this? Do you accept my insurance?

Most insurance companies require a doctor’s referral for reimbursement. Private pay does not require a referral, although I prefer to have one. Just call or make an appointment at your doctor’s office and let them know what your symptoms are. Once they fax a referral to my office, I will contact you. Contact your insurance if you would like to confirm and please see list of accepted insurance.
