Our goal is to complement your treatment plan with effective, evidence-based therapy while maintaining clear and consistent communication to support quality, continuity, and the best outcomes for our shared patients.
Referring a Patient​
Most insurance companies require a physician referral for compensation. Download our Referral Form under the Providers tab and fax to (808) 443-0552. We will contact the patient to schedule and appointment once we receive the completed referral.
